I, _________________________________(print name), will be participating in a baseball practice “activity” utilizing batting cages and other field equipment at the Myrtle Beach Pelicans LP (Myrtle Beach Pelicans Baseball) facilities on the date below.

I realize that in participating in this activity, I AM TAKING A RISK of injuring myself. I realize that there will be other persons involved in this at the same time, which will create a RISK OF INJURY OR DEATH TO ME AND TO OTHER PERSONS.

I WISH TO PARTICIPATE IN THIS ACTIVITY IN SPITE OF THESE RISKS.

In consideration of being permitted to participate, I agree to exercise care in participating so as to limit the risk of injury to myself and others, and hereby assume full responsibility for: 1) death and any injuries that may occur to me and my property as a result of my participation, and 2) any injuries to other persons or property which I may cause as a result of my participation.

I AGREE TO RELEASE AND FOREVER DISCHARGE MYRTLE BEACH PELICANS LP, its employees, officers, partners, agents and any other persons, corporations or organizations sponsoring, associated with or otherwise involved in this promotion (collectively the “Released Parties”), from any responsibility or liability for injuries that may occur to me as a result of my participation. I further agree that I WILL INDEMNIFY THE FOREGOING RELEASED PARTIES FROM AND HOLD THEM HARMLESS against all liability, loss, damages, cost and expenses, which they or anyone of them may incur or are required to pay by reason of injury or death to persons or damage to property or both allegedly caused by participating in this promotion.

I represent that I have carefully read this agreement and fully understand its contents. I am aware that this is a release of liability and hold harmless contract between myself and Myrtle Beach Pelicans LP and sign it voluntarily and of my own free will.

IN WITNESS WHEREOF, I have executed this Release at Pelicans Ballpark, Myrtle Beach, South Carolina on _______________ (date).
